Overview
- Adult day health care is a licensed program that provides organized health care for individuals of all ages during specified daytime hours, which may include: continuous supervision to ensure that health care needs are being met; supervision of self-administration of medications; nursing services that include medication administration for participants who are unable to self-administer; provision of personal care services; self-care training; and social and
- recreational activities.
- These programs may serve elderly or disabled persons
- who are 21 years of age or older, who meet Medicaid’s eligibility criteria for waiver program services, and who, without these services, would be admitted to a nursing facility.
- Should an ADHC participants condition become such that a different type of care would be more beneficial, the ADHC provider must make the necessary transfer or referral and advise the Medicaid waiver program of the referral or transfer.

Product & Services
Activities:
The program will provide a variety of activities for clients to participate in, including but not limited to community outings, holiday parties, exercise sessions, games, discussions on current events, music and dance, arts and crafts, computer training, and access to a multilingual library.
Health Services:
Clients' personal care and daily living requirements, including walking, eating, personal hygiene, and grooming, will be fulfilled based on their individual care plan, age, and abilities. Within the program, clients have the flexibility to design their own schedule, whether for a full or half day, and select the days they prefer to attend.
Nursing Services:
The nursing services at the facility will be overseen by registered nurses, who will also administer medication as needed. The nursing staff will be trained in CPR and First AID to provide emergency services, and in the event of an emergency, clients will first be treated
on-site before being transported by ambulance to a nearby hospital. Each client will have a personalized care plan, as ordered by their physician, to ensure that their daily health and personal care needs are met promptly and safely. The medication and medical records will
be kept secure in an in-house station, and the facility will follow universal precautions for infection control, as mandated by KY state regulations. All clients will be provided with incidental supplies and necessary equipment to meet their individual needs. If needed, the
administrator will arrange referrals or transfers for clients who require a different level of care.
Food Services:
Breakfast, lunch, and snacks will be served daily as needed.
Transportation:
Clients will be provided with transportation to and from ABBA ADHC for their daily personal care needs and any necessary medical appointments, including those requiring a translator. The facility will make special arrangements for clients who require a wheelchair. Private transportation may also be available for eligible clients.
