Attendant Care
Our caregivers and healthcare team are can ensuring that your

Case Management
ABBA Case Management provides case management services to
participants in the Medicaid Home and Community-Based Wavier. Our case managers are knowledgeable about wavier and community resources
and HCB regulations to ensure that all the needs of participants are met
timely and with the highest quality care.
Adult Day Care
Our team offers the facilitation of the adult day care services and the
activities, including assistance with eating, medication, exercise programs,
social activities, health monitoring, as well as counseling. Most patients
will benefit from adult day care programs because these allow them to
maintain social interactions with other people while staying in the
community. Nursing services play an important part in the care including:
appointments, medication accommodations, vital monitoring and dietary
accommodations.

How do I apply for Medicaid?
Apply online using Kynect, by calling the Department for Community Based Services (DCBS) at (855) 306-8959, or in-person at a DCBS office.
How do I switch my services?
Contact your case manager to let them know your intent to switch services to another provider.
